Even though the Medicare prescription drug benefit has provided access to medications at
less-than-anticipated cost to the government—and lower out-of-pocket spending for
seniors—many Democrats and consumer advocates want to overhaul the program. Critics contend that the
federal government can negotiate prices with pharmaceutical companies that are lower than those obtained by private
insurers operating prescription drug plans (PDPs), and some reformers charge that the program is too complex and
confusing for elderly beneficiaries, pointing particularly to the infamous "donut hole" that is
affecting more Medicare patients than anticipated.

In a nested, case-control study published in the Archives of General Psychiatry, investigators
observed that the use of selective serotonin-reuptake inhibitors (SSRIs) increased the risk of upper

Tocilizumab, an investigational agent for the treatment of moderate-to-severe rheumatoid arthritis, is
a humanized anti-IL-6 receptor monoclonal antibody. Because tocilizumab contains a mouse monoclonal antibody
grafted onto human immunoglobulin, the grafted antibody is less antigenic and has a longer half-life than the mouse
antibody. When administered, tocilizumab inhibits IL-6 activity by competing for both the membrane-bound and
soluble types of IL-6 receptors, thus eliminating IL-6 transduction into the cell.

Erythropoietin, a humoral factor produced predominantly in the kidney, stimulates red blood cell
production in the bone marrow. Erythropoiesis-stimulating agents (ESAs) have been used for years in the treatment
of anemia, with extensive experience and benefits in anemia of chronic kidney disease. Recent data have suggested
adverse consequences with use of ESAs, perhaps relating to inappropriate use, and prompting release of guidelines
to ensure safe use and maximize benefit. When prescribing ESAs, indications, requirements to monitor laboratory
parameters (hemoglobin levels and ferrokinetics), and clinical status need to be stringently
followed.

The US orphan drug program has demonstrated that economic incentives and regulatory flexibility can
spur development of treatments for small patient populations. Since enactment of the Orphan Drug Act (ODA) in 1983,
FDA has approved >300 medicines for approximately 12 million patients around the world. However, there
are approximately 6,000 to 8,000 rare diseases, so “we still have a very long way to
go,” commented Janet Woodcock, director of the Center for Drug Evaluation and Research (CDER), at a
May conference commemorating ODA’s 25th anniversary, which was sponsored by the Drug Information
Association (DIA).

According to the results of an extended follow-up of the African American Study of Kidney Disease and
Hypertension (AASK), long-term reduction of blood pressure to a lower goal than the standard demonstrates no
significant effect on the progression of chronic kidney disease (CKD) in nondiabetic hypertensive patients, except

medicine and director of the hypertension program at Case Western Reserve University, Cleveland. These results were

Clinicians should be wary of health claims made for trace elements such as iron or chromium. While
there is no doubt that trace elements are needed for health, there is significant doubt about their utility in
preventing or curing diabetes and other diseases, said Eliseo Guallar, MD, DrPH, Johns Hopkins Bloomberg School of
Public Health, Baltimore. There are many hypotheses about the beneficial effects of trace elements, but little
high-quality data, little reliable population data, few useful biomarkers, relatively crude analytical methods and
few mechanistic studies.

Several novel classes of agents designed to treat hyperglycemia are under investigation. These agents
were the subjects of a symposium here. One such novel class is the sodium glucose co-transporter (SGLT) type 2
inhibitors, which reduce glucose levels by increasing kidney excretion of glucose. The kidney plays an important
role in the handling of glucose, said Robert R. Henry, MD, professor of medicine and chief of VA endocrinology and
metabolism at the University of California, San Diego. SGLT-2 is expressed almost exclusively in the kidney. About
90% of glucose reabsorption by the kidney is mediated by SGLT-2 under normal circumstances.

Advanced age should not be a barrier to initiating adjuvant chemotherapy in patients with stage III
colon cancer. In an observational study, the oldest patients suffered no more adverse events when treated with
adjuvant chemotherapy than did younger patients but were much less likely than their younger counterparts to
actually receive adjuvant chemotherapy for their colon cancer, said Katherine L. Khan, MD, professor of medicine at
the University of California, Los Angeles.

Cancer-related fatigue significantly interferes with patients' quality of life, and some areas often
affected among these patients are not normally included on standard quality-of-life assessments, said Karen M.
Mustian, PhD, University of Rochester School of Medicine, NY. More randomized, controlled trials are needed to
